Honest answers needed

 I spoke to a doctor friend of mine retired recently from Government service and he lamented the declining standard of the nurses and the medical assistants.

Medical assistants were formerly known as Hospital assistants. In the sixties and seventies, most of the district hospitals could not be run without the HAs. They were like the barefoot doctors,. They helped to man the emergency service in rural hospitals, helped run the outpatients clinics, stitched up wounds and lacerations, set up drips, catheterised patients who could not pass urine…. In short, they were great assistants to the doctors then.

HAs are trained in government institute locally just like the nurses. My doctor friend told me that the standard now is so horrid that some of them cannot even stitch up a wound properly. Diagnosis wise, they were world apart from the old HAs/MAs.

We wondered whether the latest death in NS camp has anything to do with wrong diagnosis by the MAs in charge. If the cause of death is toxic megacolon, which is due to infection, I would like to ask a few  questions: where is the source of infections, from food or water? When did the MAs realise that this is not a simple case of abdominal colic ? The treatment of toxic megacolon (which can kill very fast and is one of the medical emergencies) depends on early diagnosis and the speedy adminstration of correct treatment which may include surgical removal of a section of the colon involved.

How good is our MAs and Nurses nowadays ?

Did we lower the passing marks for these people dealing with life and death situation? We need honest answers to really solve the declining standard of MAs and nurses. If these people are incompetent, then unnecessary deaths cannot be avoided in camps and even hospitals.

I view this as  part of the greater problems of loss of excellence in practically all aspects of Malaysian life.The examples are so numerous . Just to cite a few: 

In football, we were among the last in rankings. in hockey where once we were feared and respected, we are ow an also-run.

Our universities were ranked lower and lower; Our judiciary is in a mess (waiting for the Lingam tape report); our crime situation is much worse than 10 or 20 yrs back due to you-konw-what;

our civil service is not as efficient as before (that I suspect is one of the reasons that government has raised the retirement age; without the present badge of pengarahs and so on, the younger generations taking over cannot even speak good and decent English- how to get knowledge and ideas from other countries?)

 Despite the hundreds of all As students produced, the level of knowledge among these all As students are not even half as good as those all As in the sixties and seventies;

construction wise, houses are not built as solid as before; new schemes are full of problems like leaking roof, cracking walls; even MRR2 had to be closed for repair some time back.

What we need is to gain back the excellence that Malaysia has lost. To do it, there is no other way but to practise true meritocracy and fair competition among all races. In addition, education needs to be reviewed wholesome. Only with that, can attitude be changed and can a culture of responsibility and good work ethics be developed.

    Yet..UMNO is so proud of their achievements. What achievements? Let them answer Doc’s message..if they really care for Malaysians.
    Yes..the poor girl is dead. Few few weeks..all Malaysians will naturally forget ……leaving the poor parents …forever sad. Not to be cruel…….but how I wish…that girl is Dollah’s or Mahathir’s grand-daughter!!
    All the UMNO balls carriers ….will yell..for swift justice..and the truths of it all out….fast!

    Universities??….go listen to Anwar wonderful video speech on universities…at his blog.
    He said..UMNO malays are afraid of Chinese smart ones….so the put an idiot UMNO person ..as head of University…to make them look smart….plus much more ….direct and sensible speech.

    wow. monsterball. that was. enlightening. there goes my happy mood. Do you always write comments like that my boy? You should do something to at least censor some sentences there that can be deemed rather inappropriate for minors.

    Anyway, doc quoting my previous posting in your previous post, I am a concerned parent and I do want answers, at least for the benefit of the poor girl’s family. May her soul rest in peace. My daughter might be chosen for NS too in 2 years time, and as much as i really want her to participate for her own benefit which i do the nS is good for her too, i’ll be worried senseless if I know something horrible is going to happen to her.

    Shouldnt wehave the best MAs in the NS camps too?

    The unwritten policy of lowering marks and qualifying standards to accomodate those who do not qualified is what brings the standard of our services to such deplorable state. The more they shield the unworthy, the greater will the people suffer.

    Why didn’t he realise this when he was the president as well as the only cabinet minister from the party? why didn’t he do something then rather than just be contended to be a minister of a beggar party? Why he did not tell the members that he was like a beggar inside the cabinet?

    IS he really like Chua SOi lek said, double face?

    If along he knew about the situation that the party is like a beggar party, , then he has misled the members for more the 22 yrs he was the minister.

    Wy didnt he consider pulling the party out from BN if that is the case?

    Why still stick to the rotten ones ?

    What true meritocracy? Wonder when will this materialise? Wasnt it mentioned that the status quo of the system would be maintained? Quotas for scholarships, places in matriculation colleges and universities have always been racially inclined.

    Nowadays preU courses run by private colleges are insanely expensive. Even with their scholarships, students still have to fork out quite a sum for miscellaneous stuff.

    What choice is there left for those with financial constraints but to opt for the much cheaper but globally recognised STPM. Most students fear taking STPM considering it as one of the most difficult preU course.

    STPM is definitely mentally challenging compared to the relatively “lighter” matriculation course. My heart is always with students who despite achieving CGPA of 4 were denied their first choice of courses in their application to local universities. There was one year when 128 STPM students with the maximun CGPA 4 ended up as casualties.

    How else to acheive excellence when excellent students are denied to pursue their choice course?

    You will never get honest answers. It is so so obvious that for certain “difficult” subjects like Additional Mathematics, the marks are dumb downed to enable students to score A’s more easily.

    If the bar is raised higher it would jeopardise the chance of scholarship application. But the ridiculous thing is that it is insanely difficult nowadays to score an 1A in the ChineseLanguage paper and this has brought about a dwindling number of students taking the paper. How sad!

    aadya….Sorry to spoil your happy mood.
    My message was written with hatred …sadness and disgusted feelings.
    With those combinations symptoms……the messages is already mild …due to my respect for Doc.Hsu.
    Furthermore….if you do mix around like me..with the people on the streets …around most English speaking countries….they love my manners…points..and down to earth personality.
    Just look at how Mahathir speaks……..so refine…so cultured..so high class..not one word of foul words….yet he is the biggest con man..crooked politician..and the worst Prime Minister in the history of our country. He gets so many people…to do his dirty job…that’s how smart ..a crook….he is.
    And I seriously think…today youngsters …can read…listen and see more foul materials..music and movies….every day.
    They will appreciate me more than you.
    Calling me ‘my boy”…makes you feel big?

    If there is no total change of government….we will never know the real truths of many important issues. We will never progress.
    This is a good start….with more voices…to represent Malaysians and healthy competitions….to manage the country.
    But leave it to UMNO and BN….gone case….back to law of the jungle …..and space age…ambitions….to really confuse all Malaysians ..make us go nuts…..and they will have a field day…ruling us like slaves…laughing to the banks….with stolen money…travelling all over the world….controlling our minds and souls….to work like slaves….for them to enjoy life…like Britain…doing exactly that….just..100 years ago.
    Malaysia will be the first…most prosperous country….having Malaysians..being con…..right here ..by our own so call countrymen. The world. leaders admire Malaysians tolerance and ever forgiving characters…..but enough is enough.
    Monkies are fighting each other now. Malaysia is a better place than yesterday.

    I agree with daffodils. Private colleges are so expensive now. Even twinning is no longer availble for an average family.

    STPM is the toughest of all pre U courses. Whereas Matriculation course for certain class of citizen is like a breeze compared to it.

    HOw can you have 2 systems for PreU? How to practise true meritocracy?

    The first act to revamp the education system will definitely be doing away with these PreU streams and have one unifed stream for all.

    Get external eaminiers for SPM, PreU Exam so that no lowering of marks will be allowed.

    Even technical courses like nursing and training MAs must have external examiners, so that the standard can be maintained.

    Not all people can afford private hospitals, the fees are so high now only the super rich can afford private medical care another few years times. So most of the Malaysian will have to depend on government hospitals, and the people manning these hospitals, the first lines, are the local trained nurses and MAs.

    Doctors cannot be with the patients all the time. So what is the use of building expensive looking hospitals but staffed them with half-past-six paramedical staff?

My deepest regret to assume that by the way you write your comments, you sound like a teenager to me and as a mother that was just my maternal instinct to use the term “my boy”. My deepest regret for that MONSTERball.

    Doc, i agree with your concern with us not having enough qualified MAs in goverment hospitals. But what can we do if most of our overseas scholars choose to work abroad instead of coming back here? They really must do something about the quality of service in our hospitals. But from what I heard, the Sungai Buloh Hospital is doing pretty good as a government hospital? At least I have heard that their service and facilities are “a step above” regular government hospitals.

    Doc..In a nutshell….why our hospitals are not equpt with quality people….is basically…local universities produces low class medical students.
    It all point to the Universities.
    Did you not rad.hundreds of foreign nurses are imported…..especially from India?
    And those guys who studied overseas…to be doctors…join the government for experiences only….for few years…..thus government hospitals are full of doctors..love to poke this or that to a patient…to learn.
    Once feeling confident..they opt to start their own…or prefer to join private medical centres.
    How’s this….from an over the hill…teenager??

    aadya, you are right. Certain hospitals, especially the new ones in Klang Valley, are staffed with experinced staff. These include Sg Buloh Hospital, Selayang Hospital, Putrajaya hospital. It is not easy to get posted to these hospitals, either you have to be good, senior, or with strings.

    BUt generally, the standard of MAs and nurses have declined especially in districts and rural hospitals, and even in HKL.

    Many foreign trained Malaysian doctors came back for a while but left after becoming disillusioned with the poor working conditions and poor propests of career advancement. How many can become professors in medical faculties? Even they are very good, professorship is not awarded through meritocracy…I need not elaborate. So not just pay wise. They also think of the education aspects for their children, any fair chance of succedding for thier children and so on…. There are also so much of red tapes everyway… These are professionas wanted everywhere, so why should they tolerate unfairness, discrimination and so on… The easiest way is for them to move out again.

    Exactly,why should professional need to put up with all the “b/s” throwing at them from those racists controlled government, their services are needed in developed countries, racial discrimination in promotion and pay is at the minimum, as most of the advance countries are based on qualification and experience not race n religion like in this little arrogance islamic country, where even the citizens are considered as lower class of people by government.

    Umno put idiots as Chancellors to a Universities….to look smart . They also clearly playing … race and religions….favoring only UMNO supporters…to hold high posts….in all learning centers….so much so….private learning institutions.. are also low class.They dare not correct them…..as our own class set up by UMNO are much much lower!! What is the rank of of Educational system in the world?? They are afraid of the smart Chinese…so said Anwar.
    Mara produces so many thousands students….good for this or that..with tons of diplomas. Go interview them…and know..the quality they produced…….so terribly low.
    So you see…you need to get to the roots…to know why so many sickening things are happening in our country…death of the poor girl..and few others..all forgotten…….thousand opt to study overseas..and not in Malaysia……especially by the poor Chinese and Indian families..sweating and tolling…..their whole lives….to educate their children overseas. They are very poor….yet they opt to get their children.to study overseas. Why. Are they to showing off???? NO!! Are UMNO ministers blind? No… they are not. Their own children are studying overseas…from primary standards..and most have bought houses…..especially in Australia. WHERE DO THEY GET ALL THE MONEY??
    Trace the root cause….of anything…..and you have clear answers.
    UMNO simply want Malaysians .to be less educated……less successful..less smarter than themselves…..so they can continue to brag and apply.. control of the weak minds… with race and religion politics. They are real cunning sly foxes from hell!!
    Poor innocent will die more and more. Just watch freeway…cracked and collapse….15 year old buses..approved by UMNO..kills people….bird flu…this flu will come back…as they are lazy..and no follow ups on health matters….at foot stalls……except .when in trouble.
    Thousands of robberies…rapes….extortion cases…99% not solve.
    Traffic police is no where to be seen …on rainy days…and on Fridays…devotion to Allah….more important than serving Malaysians. Who can say that’s wrong? I say….it is totally wrong to be irresponsible…in the name of Allah!!

    What this government want to see in Malaysia is all non Malay citizens are denied equal opportunity and access to higher education, skill and better life, so generation to come can only become unskilled worker, middle and lower level administrators, forever to serve as slave or servant to those ketuanan racists in Malaysia.
